Pre-Registration Form

All supporters should upload proof of their vaccination status via the COVID-19 pre-registration form prior to attending the stadium. A link to this form will be sent to you via email, text (SMS) or letter

By completing this form, you will provide us with your COVID Pass, the status of which will be recorded and linked to your supporter number in our access control system

You will also be making a declaration that you will have a valid COVID Pass or appropriate evidence of a negative test on arrival at the stadium. If you pass your ticket on to someone else or if you buy multiple tickets for other people in a group, you will also declare that you have informed those people that they need to have a valid COVID Pass or appropriate evidence of a negative test on arrival at the stadium

There is also a section to declare that you will not attend a match if experiencing COVID-19 symptoms.

Proof of COVID-19 status

Once you have pre-registered, you still need the ability to prove your COVID-19 status on a matchday as the Government requires the Club to complete a significant number of spot checks for each match. Any supporter unable to provide evidence will risk being denied entry to the stadium. Acceptable forms of entry are as follows:

  • A valid NHS COVID Pass from the NHS App
  • A printed PDF version of a valid NHS COVID Pass
  • A text or email indicating a negative lateral flow or PCR test result taken within 48 hours of kick-off
  • An NHS COVID Pass letter
  • An EU DCC (European Union Digital COVID Certificate)

FAQs

How do I upload my COVID-19 Pass?

Log on to the NHS App or website, select ‘Domestic’ NHS COVID Pass and then download either a pdf copy to your device or receive an offline copy via email. This can then be attached to your COVID-19 pre-registration form. Please be aware you cannot download your pass from your Apple / Google wallet.

Can I use my COVID-19 vaccine cards to enter the stadium?

Government Plan B legislation states that we will not be able to accept these as evidence of vaccination status. Please download the NHS COVID Pass via the NHS App or provide a NHS Covid Pass letter. If you do not have access to a computer or smartphone, you can request a letter via phone by calling 119.

Can I use evidence of a negative lateral flow or negative PCR to gain entry into the stadium?

Yes. Test results can be reported to the NHS COVID-19 App or via https://www.gov.uk/report-covid19-result where you will receive a text or email notification that can be checked on entry to the stadium.

Can I use my lateral flow plastic test bed as evidence?

No. Negative lateral flow tests (within 48 hours of the fixture) need to be reported via the NHS COVID-19 App or via https://www.gov.uk/report-covid19-result where you will receive a text or email notification that can be checked on entry.

What happens if I don’t have a smartphone?

You can obtain a COVID Pass letter using this link or you can request a letter via phone by calling 119. Please make sure you have a copy of your letter when you attend the stadium. Alternatively, you can report a negative lateral flow test or PCR obtained within 48 hours prior to kick off via the Government website and show your text or email notification upon entry.

What happens if I am exempt?

After December 25, 2021, you can apply for and receive your COVID Pass medical exemption by calling 119. You can use the domestic NHS COVID Pass and it won’t show that you have a medical exemption, just that you have the NHS COVID Pass. This can then be downloaded and attached to the COVID-19 pre-registration form.

What happens if I turn up to the stadium and I haven’t pre-registered but I have proof of vaccination?

If the proof of vaccination is via the NHS Pass or an NHS COVID Pass letter then you will be able to enter the stadium but if you have not pre-registered then your season ticket / card may be deactivated for future fixtures.

What happens if I turn up to the stadium and I haven’t pre-registered and I haven’t got any evidence of COVID-19 vaccination or a negative test?

The Government legislation that came into effect as of 15 December 2021 states that you risk being denied entry to the stadium.

If I have pre-registered, do I still need to bring evidence of my COVID-19 status to the stadium on a matchday?

Yes. We will be conducting spot checks and you will need to bring any of the following with you:

  • A valid NHS COVID Pass from the NHS App
  • A printed PDF version of a valid NHS COVID Pass
  • A text or email indicating a negative lateral flow or PCR test result taken within 48 hours of kick-off
  • An NHS COVID Pass letter
  • An EU DCC (European Union Digital COVID Certificate)

Once I have pre-registered once, do I need to pre-register for other fixtures?

No. You only need to fill in the pre-registration form once, unless your vaccination status changes. Then you will need to go back onto the form and update your details.
