How to upload your COVID-19 pass to Norwich City's pre-registration form

For Norwich City’s match agains Everton on Saturday and subsequent fixtures, it will be a requirement for supporters to provide proof that they are vaccinated against COVID-19 or have received a negative lateral flow test 48 hours before a game.

Fans should upload proof of vaccination to the pre-registration form which will be sent directly via email, text message or post.

Alternatively, you can fill in the form HERE.

Fans will only need to do this once, however the link will remain open if they need to update their vaccination status in the future.

See below a guide on how to upload your COVID-19 pass to the pre-registration form:

  • Download NHS App on mobile, or visit https://covid-status.service.nhsx.nhs.uk/
  • Follow the steps to register your details
  • Once logged in, those who have been fully vaccinated for two weeks will be able to access their COVID pass by selecting ‘NHS COVID Pass’
  • Select ‘Domestic’
  • Scroll down and select either ‘Download PDF copy’ or ‘Receive an offline copy by email’ which will ensure the pass is saved to your device as a file
  • Click the pre-register form link sent to you via email/SMS
  • Upload your COVID pass to the form

Please be aware that you cannot download your COVID pass from your Google or Apple wallet on your mobile device.

Failure to upload your COVID pass to the pre-registration form may result in your ticket being deactivated for a fixture. The club strongly recommends supporters complete their pre-registration form well in advance of their attendance at our fixtures.
